AREDS2 Vitamins for Macular Degeneration: Who Should Take Them?
Walk through the vitamin aisle and you will find plenty of supplements promising to support eye health.
For people with age-related macular degeneration, however, one specific formulation has much stronger evidence behind it: AREDS2.
AREDS2 supplements contain a particular combination of antioxidants and minerals studied by the National Eye Institute. For people with certain stages of AMD, taking the formula regularly helps reduce the risk of the disease progressing to more advanced forms.
But AREDS2 is not appropriate for everyone.
It does not prevent AMD from developing, does not reverse retinal damage, and has not been shown to benefit people with no AMD or only early AMD.
Whether you should take AREDS2 depends largely on what stage of macular degeneration you have.

1. What does AREDS2 mean?
AREDS stands for the Age-Related Eye Disease Study.
The National Eye Institute conducted two major clinical trials known as AREDS and AREDS2 to understand whether nutritional supplements influence age-related macular degeneration.
The original AREDS study found that a specific high-dose combination of antioxidants and zinc reduced the risk of progression from intermediate to advanced AMD.
AREDS2 then studied changes to the original formula.
The newer formula replaced beta-carotene with the carotenoids lutein and zeaxanthin, which are naturally present in the retina.
The AREDS2 formulation is now generally preferred.
2. Who benefits from AREDS2 vitamins?
AREDS2 supplements are mainly recommended for people with intermediate AMD in one or both eyes.
They are also useful for certain people who have late AMD in one eye but not the other, because the supplements help reduce the chance of the less-affected eye progressing.
In simple terms, AREDS2 is most useful when AMD has progressed beyond the earliest stage but there is still retinal tissue worth protecting from further progression.
Your retina specialist determines whether your retinal findings fit the stage shown to benefit in the AREDS studies.
3. Who does not benefit from AREDS2?
AREDS2 is not a general eye vitamin for everyone.
Research has not shown the same benefit for people who:
do not have AMD
have only early AMD
want to prevent AMD from developing in the future
Taking AREDS2 because AMD runs in your family does not mean you are preventing the disease.
Likewise, seeing a few drusen during an eye examination does not automatically mean the AREDS2 formula is appropriate.
The amount, size, and pattern of drusen, pigment changes, and condition of each eye all matter.
This is why it is better to ask your eye doctor which AMD stage you have before starting supplements.
4. What is actually in the AREDS2 formula?
The standard AREDS2 formula studied by the National Eye Institute contains:
Vitamin C: 500 mg
Vitamin E: 400 IU
Zinc: 80 mg
Copper: 2 mg
Lutein: 10 mg
Zeaxanthin: 2 mg
These are substantially higher amounts than those found in an ordinary multivitamin.
The formula is designed as a specific combination rather than a collection of separate general supplements.
When buying an AREDS2 product, check the ingredient list rather than relying only on phrases such as “eye health formula.”
Not every product marketed for eye health contains the studied AREDS2 amounts.
5. Why are lutein and zeaxanthin included?
Lutein and zeaxanthin are carotenoids concentrated naturally in the macula.
They form part of the macular pigment and help filter light and protect retinal tissue from oxidative stress.
The original AREDS formula contained beta-carotene instead.
AREDS2 researchers studied whether replacing beta-carotene with lutein and zeaxanthin would maintain the benefit while improving the safety profile.
The newer formulation became particularly important for people who smoke or previously smoked.
6. Why should smokers avoid beta-carotene?
This is one of the most important distinctions between the original AREDS formula and AREDS2.
High-dose beta-carotene supplements were associated with an increased risk of lung cancer in people who smoked or had previously smoked.
The AREDS2 formula does not contain beta-carotene.
Instead, it contains lutein and zeaxanthin.
If you currently smoke or smoked in the past, avoid older AREDS products containing beta-carotene and discuss supplement choice with your doctor.
Checking the label matters because older formulations and other supplements containing beta-carotene are still available.
7. How much does AREDS2 reduce AMD progression?
The AREDS studies found that the supplement formulation reduced the risk of progression from intermediate to advanced AMD by roughly 25% over five years in the groups shown to benefit.
That number is important, but it needs to be interpreted correctly.
AREDS2 does not eliminate the risk of progression.
It lowers the risk.
Someone taking AREDS2 still needs regular retinal examinations and monitoring.
The vitamins work as part of AMD management rather than as a substitute for eye care.
8. Does AREDS2 improve vision?
AREDS2 is designed to slow disease progression, not make existing retinal damage disappear.
You should not expect your reading vision to suddenly sharpen after starting the supplements.
They do not:
remove drusen
reverse geographic atrophy
restore damaged retinal cells
cure macular degeneration
reverse scarring from wet AMD
Their benefit is preventative in the sense of slowing progression in people who already have the stages of AMD shown to benefit.
That makes AREDS2 very different from a medication designed to produce an immediate symptomatic improvement.
9. Does AREDS2 treat geographic atrophy?
Geographic atrophy is an advanced form of dry AMD in which areas of retinal cells gradually disappear.
AREDS2 does not restore these areas.
For certain patients, AREDS2 still forms part of broader AMD management depending on the stage and condition of the other eye.
However, newer complement-inhibitor injections are now available specifically to slow progression of geographic atrophy in selected patients.
The two approaches serve different purposes.
Your retina specialist looks at each eye individually when deciding which treatments are appropriate.
10. Does AREDS2 treat wet macular degeneration?
No.
Wet AMD develops when abnormal blood vessels grow and leak fluid or blood around the macula.
The main treatment is usually anti-VEGF medication injected into the eye.
AREDS2 does not stop active blood-vessel leakage and cannot replace retinal injections.
Someone receiving wet AMD injections might still be advised to take AREDS2 depending on the AMD stage in the other eye or other retinal findings.
The supplement and injection treatment address different parts of the disease.
11. Can AREDS2 prevent AMD?
No.
This is a common misunderstanding.
AREDS2 has not been shown to prevent AMD in someone who does not already have the stage of disease shown to benefit.
If you have no AMD, the most useful approach is focusing on established modifiable risk factors and maintaining regular eye care.
Important habits include:
not smoking
staying physically active
eating a balanced diet
managing blood pressure
managing cholesterol
maintaining appropriate cardiovascular care
keeping recommended eye appointments
Supplements should not replace these measures.
12. What if you only have early AMD?
If your eye doctor says you have early AMD, AREDS2 is generally not recommended solely to prevent progression to intermediate disease because clinical trials did not show a clear benefit at that stage.
Instead, monitoring becomes important.
Your eye doctor might recommend:
regular dilated examinations
retinal imaging
monitoring central vision
healthy lifestyle habits
If AMD progresses to an intermediate stage, AREDS2 might then become appropriate.
This is another reason knowing your actual AMD stage matters more than simply knowing you have “some macular degeneration.”
13. What if you have late AMD in one eye?
This is one of the situations where AREDS2 might be particularly useful.
If one eye already has late AMD and the other eye has intermediate AMD, the formula helps reduce the risk of the less-affected eye progressing.
Protecting the better-seeing eye often becomes a major priority.
Your retina specialist looks at both eyes separately rather than assigning one overall AMD stage to the person.
14. Can you take a regular multivitamin instead?
A standard multivitamin is not equivalent to AREDS2.
The AREDS2 formulation uses specific doses of vitamins and minerals which are substantially higher than those normally found in daily multivitamins.
A general multivitamin might still be appropriate for other health reasons, but it does not provide the same studied AMD benefit.
Likewise, taking individual vitamin C, vitamin E, zinc, or lutein supplements separately does not necessarily reproduce the AREDS2 formulation.
If AREDS2 is recommended, look for a product matching the studied formula.
15. Can food replace AREDS2?
A nutritious diet is important for overall eye and cardiovascular health.
Foods containing lutein and zeaxanthin include:
spinach
kale
collard greens
broccoli
peas
other green vegetables
A balanced diet also provides many nutrients not included in AREDS2.
However, the high doses used in the AREDS2 trials are difficult to reproduce through food alone.
For someone with intermediate AMD who has been advised to take AREDS2, eating a healthy diet is in addition to, rather than a replacement for, the studied supplement.
16. What about omega-3 or fish-oil supplements?
AREDS2 researchers also studied omega-3 fatty acids.
Adding omega-3 supplements to the AREDS formula did not provide additional protection against progression to advanced AMD in the trial.
This does not mean fish or omega-3-rich foods are unhealthy.
A diet including fish fits well within broader recommendations for cardiovascular and eye health.
It simply means omega-3 supplements are not part of the standard AREDS2 formula for slowing AMD progression.
17. Why is copper included?
AREDS2 contains zinc at a much higher dose than an ordinary diet or multivitamin.
High levels of zinc sometimes interfere with copper absorption.
Copper is therefore included in the formula to reduce the risk of zinc-related copper deficiency.
This is another example of why AREDS2 should be viewed as a carefully studied formulation rather than a random collection of vitamins.
18. Are there side effects or medication interactions?
AREDS2 contains high doses of several nutrients.
Supplements sometimes:
cause digestive upset
interact with medications
affect how other nutrients are absorbed
Your primary-care clinician should know about every supplement you take, especially if you use several medications or have other medical conditions.
Do not assume that something sold over the counter is automatically appropriate simply because it is labeled as a vitamin.
19. How do you choose an AREDS2 supplement?
The National Eye Institute does not endorse a specific commercial brand.
Instead, check whether the product matches the studied formulation.
Look for approximately:
vitamin C 500 mg
vitamin E 400 IU
lutein 10 mg
zeaxanthin 2 mg
zinc 80 mg
copper 2 mg
Avoid formulas containing beta-carotene if you currently smoke or previously smoked.
If the label is confusing, bring the bottle or a photograph of the ingredients to your retina appointment.
20. More is not better
Because AREDS2 contains vitamins and minerals, it is easy to assume taking extra will offer additional protection.
There is no evidence that increasing the doses beyond the studied formula improves AMD outcomes.
Higher doses also increase the potential for unwanted effects.
Stick to the formulation and dosage recommended by your healthcare team.
21. AREDS2 does not replace retinal monitoring
Even someone taking AREDS2 consistently still needs regular retinal examinations.
Dry AMD sometimes progresses gradually.
It also sometimes converts to wet AMD, which requires a completely different treatment approach.
Watch for new symptoms including:
straight lines becoming wavy
a new dark central spot
sudden central blur
missing areas of vision
rapidly worsening distortion
These symptoms deserve prompt retinal assessment.
Do not wait for the vitamins to correct a sudden visual change.
22. Use an Amsler grid if your retina specialist recommends one
An Amsler grid is often used to monitor central vision at home.
Look at the center point with one eye covered.
Repeat with the other eye.
Contact your eye doctor if you notice a new area where:
lines look bent
lines are missing
the grid looks distorted
a dark spot appears
Home monitoring does not diagnose AMD progression.
It helps identify changes between appointments.
Who should ask about AREDS2?
It is especially worth discussing AREDS2 with your retina specialist if:
you have been told you have intermediate AMD
you have significant drusen
you have late AMD in one eye
you are unsure what stage of AMD you have
you currently take an “eye vitamin” but do not know whether it matches AREDS2
you smoke or previously smoked and currently take a supplement containing beta-carotene
Your doctor will determine whether the evidence supporting AREDS2 applies to your retinal findings.
What this means for your eyes
AREDS2 is one of the best-studied nutritional supplements in ophthalmology, but its benefits apply to specific patients.
It helps reduce progression to advanced AMD in people with intermediate AMD and certain people with late AMD in one eye.
It does not prevent AMD from developing, does not provide a proven benefit for early AMD, and does not restore retinal cells already lost.
The formula also does not replace treatment for wet AMD or newer treatments for geographic atrophy.
The most useful first step is knowing what stage of AMD you have.
Once that is clear, your retina specialist can tell you whether AREDS2 deserves a place in your long-term eye-care plan.
